Please review and agree to each of the following requirements by checking the corresponding boxes below.

I will comply with all Health Department, Fire Department, and Department of Agriculture guidelines, restrictions, and inspections. I understand that I may be asked to demonstrate compliance with these guidelines at any time.
As a vendor, I am responsible for obtaining a Town of Pulaski Business License (unless you already have one for the County of Pulaski), as well as the collection, reporting, and paying of sales and prepared foods taxes to the Commonwealth of Virginia. Applicable forms are available at the Town Municipal office.
I understand that each vendor is encouraged, but not required, to carry a liability policy. As the host of the Marketplace, Pulaski on Main (POM) carries a policy to cover Pulaski on Main-sponsored events; however, this coverage does not extend to the individual vendors and their products. I understand that, should I choose not to obtain insurance, Pulaski on Main will not be held responsible or liable as it relates to my individual items being sold, carried, stored, or otherwise maintained in my space, person, and/or vehicle should any of these items or employee/owner’s neglect cause injury or damage to any person(s) or property or any other situation in which damages could be sought. Pulaski on Main recommends that each vendor consult an insurance agent or company. Food trucks are required to carry an insurance policy.
All goods sold must be grown, produced, and/or handcrafted by the vendor. No resale items will be permitted. All goods must be produced within a 75 mile radius of the Marketplace. I certify that I meet these requirements. Please note that special exemptions may be granted as applicable.
I agree to supply my own table and tent. I understand that the standard 10x10 tent size is permitted. I agree to safely secure my tent to the ground to withstand wind, etc.
I agree to supply Pulaski on Main a breakdown of my profits within 24 hours after each event.
